Visa Forms
A complete listing of the required forms is available at http://www.ttsusvisas.lk/. All applicants must bring a completed Form DS-156 to their interview. Many male applicants require the Form DS-157 and other applicants may require the Form DS-158.
- Electronic Visa Application Form--must be filled out online at http://evisaforms.state.gov/. Required for all applicants, printed on white photocopy quality paper
- Supplemental Non-Immigrant Visa Application DS-157 (required for male applicants between the ages of 18 and 45 years old)
- Contact Information and Work History for Non-Immigrant Visa Applicants DS-158 (required for certain job-related applicants)
Note: If you cannot open the DS-157 and DS-158 simply by clicking on them, we suggest you right-click and select "SAVE." Save the file on your computer and then open the file directly from there.
