U.S. PASSPORTS

 Replacement for lost/stolen Passports

(US citizens sixteen years of age and above)

If a US Passport is lost or stolen please report it to the nearest Police Station and obtain an extract of your statement. The following documents are required by the Consular Section to accept your application for a new US Passport:

1. The extract of the Police Report

2. Completed DS-11 (Passport Application)

3. Completed DS-64 (Statement regarding lost/stolen passports)

4. Proof of Citizenship (certified copy of birth certificate, certified copy of lost/stolen      passport)

5. Two Photographs, which are:   

  • 2x2 inches in size
  • Identical
  • Taken within the past 6 months, showing current appearance
  • Color
  • Full face, front view with a plain white or off-white background
  • Between 1 inch and 1 3/8 inches from the bottom of the chin to the top of the head
  • Taken in normal street attire
  • Uniforms should not be worn in photographs
  • Do not wear a hat or headgear that obscures the hair or hairline unless worn daily for religious purposes
  • If you normally wear prescription glasses, a hearing device, wig or similar articles, they should be worn for your picture
  • Dark glasses or nonprescription glasses with tinted lenses are not acceptable unless you need them for medical reasons. A medical certificate may be required.

6. Fee of US$100.00 (Rs.11,500.00) FEES ARE PAYABLE IN CASH ONLY

8. Please visit the Embassy on a Monday or Wednesday between 2-4 pm

PLEASE DO NOT SIGN THE APPLICATION. IT HAS TO BE SIGNED IN THE PRESENCE OF A CONSULAR OFFICER OF THE UNITED STATES.
